Aim: In this study, the agents causing neutropenic fever in patients with acute leukaemia were determined and the applied treatment protocols and their results were discussed. Method: For this purpose, totally 84 febrile neutropenic attacks seen in 40 patients (33 AML, 7 ALL) diagnosed with and treated for acute leukemia at Haematology Clinic in Yüzüncü Yıl University, Medical School, Internal Medicine Division between July 1997-February 2002 were prospectively and retrospectively evaluated. Results: Microbiological diagnosis was obtained in 13 attacks (15.4%), 7 of them being with and 6 without bacteremia. Gram-positive bacteria were isolated in 57.1% of blood cultures and gram-positive bacteria in 42.9% of them. It was seen that coagulase-negative staphylococci (30.8%) and Escherichia coli (30.8%) were the most frequent isolated species out of gram-positive and negative bacteria. When fever origin was investigated, while clinically probable infectious origin was proven in 40 attacks (47.6%), infectious origin could not be determined in 31 attacks (36.9%), so these cases were accepted as fever of unknown origin. Initial treatment consisted of cefepim + amikacin in 44 attacks (52.4%), ceftazidim + amikacin in 25 attacks (29.8%), and imipenem + amikacin in 15 attacks (17.8%). Vancomicin was added to the treatment in 37 attacks (44%), teicoplanin in 11 attacks (13.1%) and amphotericin-B in 11 attacks (13.1%). While the response rates to the initial treatment were respectively 41%, 40% and 33.3%, response rates with treatment modifications were found respectively as 75%, 72% and 71.4%. Conclusion: We conclude that to begin suitable empiric regimen followed by clinically close follow-up and to perform suitable modifications increase the response rates at a large extent.
Keywords: Febrile neutropenia, neutropenia, acute leukemia, empiric treatment.Amaç: Bu çalışmada akut lösemili hastalarda nötropenik ateşe neden olan etkenler belirlenerek, uygulanan tedavi protokolleri ve tedavi sonuçları tartışıldı. Yöntem: Bu amaçla Temmuz 1997 - Şubat 2002 tarihleri arasında Yüzüncü Yıl Üniversitesi Tıp Fakültesi, Hematoloji Kliniğinde akut lösemi tanısı ile tedavi edilen 40 hastada (33 AML, 7 ALL) toplam 84 febril nötropenik atak prospektif ve retrospektif olarak değerlendirildi. Bulgular: Mikrobiyolojik tanı, 7’si bakteriyemili, 6’sı bakteriyemisiz olmak üzere 13 atakta (%15.4) kültür yöntemleri ile konuldu. Kan kültürlerinin 4’ünde gram pozitif, 3’ünda gram negatif bakteriler izole edildi. Gram pozitif bakterilerden en sık etkenin koagülaz-negatif stafilokoklar (4/13), gram negatif bakterilerden ise en sık etkenin Escherichia coli (4/13) olduğu görüldü. Ateş nedeni araştırıldığında; 40 atakta (%47.6) olası infeksiyon odağı klinik bulgulara dayanarak saptanırken, 31 atakta (%36.9) infeksiyon odağı saptanamadı. Başlangıç tedavisi olarak 44 (%52.4) atakta sefepim + amikasin, 25 (%29.8) atakta seftazidim + amikasin, 15 (%17.8) atakta imipenem + amikasin kullanıldı. 37 atakta (%44) tedaviye vankomisin, 11atakta (%13.1) teikoplanin, 11 atakta (%13.1) amfoterisin-B eklendi. Başlangıç tedavisine cevap oranları sırasıyla; %41, %40 ve %33.3 iken, modifiye tedavilere cevap oranları, sırasıyla %75, %72 ve %71.4 olarak gerçekleşti. Sonuç: Akut lösemili hastalarda gelişen nötropenik ateş ataklarında, uygun ampirik tedavi başlanması ve sonrasında yakın klinik takip ile uygun modifikasyonların yapılması, tedaviye cevabı büyük oranda arttırmaktadır.
